The Turkish Economic and Social Studies Foundation (TESEV) Data Owner Application Form

You may send your requests within the scope of your rights defined in Article 11 of Law no 6698 on Protection of Personal Data (the “Law”), and  as per Article 13 of the Law and Article 5 of the Communiqué on Procedures and Rules of Application to the Data Controller, along with this form through one of the four methods described below.

You can submit your request in person with wet signature or via Notary Public, to the address Vişnezade Mah. Prof. Dr. Alaeddin Yavaşça Sok. Efe Apt. No:6/9 34357 Beşiktaş, Istanbul by writing “Information Request within the Scope of the Law on Protection of Personal Data” on the letter/notice or by sending an email to info@tesev.org.tr from your e-mail or registered electronic mail (KEP) address by typing “Information Request on Protection of Personal Data” in the subject section.
